LIFESTYLE MODIFICATION PROGRAM

In May 2010 the Department of Health and Ageing (DoHA) allocated additional funds to support the implementation of the Lifestyle Modification Program (LMP) for people at risk of type 2 diabetes. The Reset Your Life program provides education and support to your patients. DoHA has funded the program for five years.

The evidence based accredited program aims to modify the risk factors and to delay or prevent the onset of Type 2 diabetes. After three years post participation in a Lifestyle Modification Program, it has been shown that fifty-eight percent of patients at risk of developing type 2 diabetes have avoided the onset of the disease.

Education is provided on lifestyle choices including diet, physical activity and stress management. Patients with newly diagnosed or existing diabetes are not eligible for this item.

The patient will be required to attend seven sessions over a six month period. Courses will be held in Mackay, Sarina and the Whitsundays. A one-off fee of $50 will be required to participate in the program (health care card or concession card holders are exempt from this fee). Face-to-face and telephone support will be provided to participants throughout the program.

Patient Eligibility Criteria

If your patient is not eligible for subsidised Lifestyle Modification Program then you can consider a referral to the MDGP LMP Reset Your Life as a non-subsidised patient. Your patient will be required to cover the cost to participate which is $280 for the seven sessions, or $40 per session (private health fund rebates may apply).

Claiming

MBS claim under the time-based health assessment items:
Item 701: Brief
health assessment of less than 30 minutes OR
Item 703: Standard health assessment of 30-44 minutes OR
Item 705: Long health assessment of 45-59 minutes OR
Item 707: Prolonged health assessment 60 or more minutes OR
Item 715: Aboriginal and Torres Strait Islander health assessment

Where appropriate, arrange referral to the Mackay Division of General Practice accredited lifestyle modification program - Reset Your Life.

Fax the GP referral form to MDGP on F: 4942 6200

A $50 co payment applies to patients referred to the LMP Reset Your Life excluding healthcare card holders and pensioners.
