MENTAL HEALTH NURSE SERVICE
The Mackay Division of General Practice is pleased to introduce the mental health nurse service as part of our allied health service.
GPs and private Psychiatrists can refer patients who have acute or chronic mental health problems and are at risk hospitalisation or have already been in hospital and require further maintenance care. Patients can access a mental health assessment, medication education and, administration, lifestyle, relationship or family counselling, crisis intervention or help with accessing other resources. Individuals and group sessions can be provided from the consulting room at the Pioneer Valley Private Hospital, an appropriate public venue and private homes.
HOW TO REFER
For all age groups - Children, Youth, Adults, Older Adults

Referral Criteria
- Consent obtained
- Under a GP or Psychiatrist care
- At risk of future hospitalisation or had a hospital episode
- Social, personal or occupational function significantly affected
